Our Services
What is Collaborative Care?
- We support Primary Care Physicians, Physician Assistants, Nurse Practitioners and Clinical Nurse Specialist.
- The care manager works with the PCP. S/he is trained to deliver evidence-based care coordination, brief behavioral interventions, and support the treatments initiated by the PCP.
- The primary responsibility of the psychiatric consultant is making treatment recommendations to the PCP, including developing treatment strategies and medical management of any complications associated with treatment.
